HEALTH & SAFETY 

The Health and Safety of our students is of paramount importance to us, which is why each parent / carer is required to fill in a detailed Health and Safety Form, along with a Photography and Filming Consent form at the start of each term. ​

Health Declaration - Under 18s
  • Please fill out the following health declaration form for each student enrolling with Levy Little Theatre classes / course / workshops.
  • Please let Levy Little Theatre know of changes in student circumstances regarding health, especially with regards to Covid-19
  • No student should attend Levy Little Theatre workshops if they display any symptoms of Covid-19. Symptoms include; persistent cough, shortness of breath, change in sense of taste or smell, a fever
Has the person enrolling for a LLT place had a cough in the last 14 days?
Has the person enrolling for a LLT place experienced shortness of breath in the last 14 days?
Has the person enrolling for a LLT place experienced a temperature at or above 37.8c in the last 14 days?
Will the LLT student have been abroad withing 14 days of their start at LLT?
Within the last 14 days has the LLT student been in contact with anyone who has Covid-19 or symptoms of Covid-19?

Photography / Filming Consent

At Levy Little Theatre we:

  • recognise the need to ensure the welfare and safety of all workshop participants, including and especially children.

  • have written a policy statement which sets out how photographs and videos of children involved in our activities should be taken and shared. A copy of this is available on this page.

  • will take all steps to ensure images are used solely for the purposes they are intended. If you become aware that these images are being used inappropriately you should inform Gemma North immediately.

  • will not take or share photographs, video or other images of anyone, including children and young people without the consent pf person/s involved and their parents or carers where applicable. This agreement fits with our overarching online safety policy.

Do you agree to photographs / videos of you (Adults Class)/your child being used on the Levy Little Theatre website, as long as LLT gain your permission first?
Do you agree to photographs / videos of you (Adults Class)/your child being used on Levy Little Theatre social media
I am aware that a copy of the Levy Little Theatre photography and filming policy is available to me on thier website.
I am aware of the potential risks associated with the use and distribution of images.
I am aware of how images will be stored in the organisation.
I agree to Levy Little Theatre and others taking wide angle, general photographs / videos during or at specific points during LLT events.
I agree that I must not share photographs / videos taken during Levy Little Theatre events on social media.
I agree that if I share images of me (Adults Class)/my child with friends and family, I should check the privacy settings of my social media account first to understand who else will be able to view these images.
